Atrial tachycardia ablation in surgically treated congenital heart disease.

چکیده

Os autores descrevem o caso de uma doente de 57 anos, com comunicação interauricular sinus venosus do tipo veia cava superior operada, submetida a estudo eletrofisiológico com o sistema de mapeamento CARTO ® 3 (Biosense Webster, J&J) por episódios de taquicardia recorrentes. A doente encontrava-se em taquicardia de QRS estreitos, com ciclo de 310ms e sequência de ativação concêntrica no seio coronário (figura 1A e 1 B), sugerindo taquicardia auricular (TA) com origem na aurícula direita (AD). Procedeu-se ao mapeamento eletroanatómico de alta densidade da AD com cateter PentaRay (figura 2C). O mapa de ativação (figura 2B) da TA sugere um mecanismo de microrreentrada -ativação centrífuga com foco na parede lateral da AD (figura 2B), coincidente com a border zone da cicatriz, (figura 2C) e visualização de ≥ 75% do ciclo da taquicardia no cateter PentaRay (figura 2A). Aplicação de radiofrequência com cateter de ablação Smartouch na região de ativação mais precoce com conversão a ritmo sinusal (RS).
